Question about custody and rights to information

Hello all.

I am curious first and foremost what my legals rights are as it pertains to information.

I need to file for my taxes and I need my children's SSN in order to do this. I did at one point in time have this information, but my ex has since taken it, and refuses to give it to me. My question is this. What are my legal rights when it comes to obtaining information like this from her?

For my second and more important question; I want to go for custody of my Children as I feel my ex is very incappable of raising our children (who are 3 and 1).

Here is the story; My ex lives with my parents (yes my parents, really). The 3 of them (my ex and 2 sons) live in a basement that is not finished with exposed insulation and wires, that constantly molds. My children are constantly sick because of this and I fear for their health. They are never bathed or clean when I pick them up for my visitation. More so, my ex does not work, she collects 400 in child support form me each month and uses this to live (note this is NOT spousal support). I feel at this point that my childrens health is dependant upon me and would like to have custody of them. So my question is this; Given this situation, what is my best chance of getting Custody? I have already called CPS and filed a report, but they do not seem to care!
