Wording for DFAS

I'm sure this has been asked on here before but is there an example anywhere of a divorce decree for us military ppl? I'd like to see how exactly the paragraph for retirement goes. I've talked to DFAS and I've got the actual wording with the formula and all, just wondering where it goes in the decree and if it's really just that simple paragraph.

If your divorce decree contains the proper language that is all that is required.
