Back Child Support

Hi....Once again I need some advice. My ex was paying child support and in November of last year he said he was having money problems and couldn't pay the full support for a couple of months. I said ok and we determined what he would pay which was alittle more than half. He paid that amount for November 06, December 06 and January 07. His regular child support resumed in February 07. He promised he pay me what he owed me with his tax return and I said that would be fine. Well, tax time came and went and he didn't pay me the money he owed me. Now, it's a year later and he still hasn't paid me and as far as I can tell, has no intent to pay me. What forms can I fill out to get this money back and get his wages garnished? I'm done being nice and have waited patiently, but no more. I'd like to collect the $1100 he owes me before he leaves the country again to go work in Iraq as a contractor. Thanks for any help!
