I don't think you are mandated to provide ANY insurance, as long as the doctor is okay with it. I have BCBS, I can go to a doctor and say I want to pay cash, do not use my insurance. My dental insurance is so bad that when I had eleven exemptions, I did that at a dentist clinic, and the amount I was to pay was less than my co-pay. Anyway, I do not think anyone is doing anything "wrong", but he may have had to pay a co-pay that would have been covered by the SF's insurance...dumb but not illegal.
