Need to amend a form...HOW?

I need to amend a FL-142 form that was filed when a legal separation was first done. I'm doing this myself(no lawyer) so wasn't aware I couldn't do this when I re-did that form with updated debt/asset values. I was told by a paralegal at the court it had to match the original one on file so I'm told I need to have the amended one notarized by the spouse.
Is there a form I do this with? just have the fl-142 notarized?
BTW - I would just use the original one on file but there are alot of problems with the original values used.
