Yes there is the "Affidavit of the Defendant" where he submits to the jurisdiction but on the list of what you have to fill out they have both of those forms (the summons w/notice and the summons w/verification of complaint). This is why I'm confused.
I have filled both of them out but its difficult to fill in the rest of the papers until I know which of the two is being used. There are boxes here and there where you have to indicate which of the two forms was used.
