My ex has supervised visits due to some undealt with mental issues. He also has done nothing to take care of them. He sees the children at each visit but rarely calls, never pays support, never paid for medical bills and never inquires about their schooling, etc. Their current stepfather has been helping me take care of the children for the last couple of years. What is the likelihood that if something happens to me the stepfather could keep custody of the children? How do I prove he is unfit so that his parental rights are removed? I am not trying to replace their father but am concerned he is doing more harm, or could do more harm, if something would happen to me.

No chance unless your ex gives up all rights, and your husband adopts them at the same time....and it would have to be before you passed on.
